New Patients:

If your client is new to medical weight loss or has not used it for at least 2 months, they have
specific package options available:
Semaglutide
3 Months (12 weeks) Supply: Ideal for new patients or those who haven't used medical weight
loss recently.
7 Week Program: An introductory package for patients unable to invest in the 3-month supply.
Note: Must allow recurring monthly charges for monthly maintenance.
1 Year Supply: Available for new patients, providing a longer-term option.
Recommendation: Following the 7-week program, it's advised to opt for two monthly
maintenance packages to equal a 3-month supply for optimal results. (Or more as needed,
some folks need to be on it for a year)

Returning Patients:

After completing a 3-month supply, returning patients have several options:
2nd Major Dose: Split into an 8-week regimen instead of 12, suitable for those wanting to
continue weight loss or haven't reached their goal.
3rd Major Dose: only available post 2nd major dose
Monthly Maintenance: Available after completing the initial 3-month supply or 7 week program
*Monthly Maintenance Consideration: Opting for monthly maintenance for three months
straight may lead to tolerance compared to the varying dosages in a 3-month supply.*
Goal Achievement:
Met Goal Weight: Patients can switch to monthly maintenance to maintain weight and suppress
hunger.
Not Met Goal Weight: It's recommended to proceed with the 2nd and 3rd major dosages, both
priced equally.
Major Weight Loss Goal: Patients aiming for significant weight loss can alternate between the
2nd and 3rd major dosages to prevent tolerance.

Tirzepatide Patients:
For clients new to medical weight loss or not using it for at least 2 months, the following
package is available:
3 Months Supply: Currently includes one month free (4 months total).
Goal Achievement:
Met Goal Weight: Transition to monthly maintenance for weight management and hunger
control.
Not Met Goal Weight: Proceed to the 2nd and 3rd major dosages, both priced equally.
Major Weight Loss Goal: Similar to other packages, alternating between the 2nd and 3rd major
dosages is recommended to avoid tolerance.

Standard Operating Procedure (SOP) for Order Completion via app.rewakenlife.com
Objective: To guide partners through the process of completing an order on our platform and ensuring a
smooth transition from purchase to fulfillment.

1. Pricing Setup: Starting 5/1/2025 all pricing is set and non-negotiable. 

2. Order Entry:
Proceed to the "Orders" section.
Select "New Order" to initiate the order entry process.
Enter the patient's information accurately, ensuring all fields are completed.
The phone number provided by the patient will be used to send a link for completing intake and consent
forms relevant to the purchased package. (You CAN select multiple options)

3. Download app and Form Completion and Payment:
The patient will receive the link by text and email to doanload the app and complete intake and consent forms.
Once the forms are completed, the patient will be directed to a payment page. (your computer screen on
app will show paperwork complete)
- The patient can choose to pay via Credit/Debit card or utilize financing services such as Affirm or CareCredit.
- Once payment is made system will change to "Paid" The button will then change to finalize

4. Finalization by Patient Coordinator:
The Patient Coordinator (partner) will no longer need to finalize as app will automaticly finalize and notify all, and then send patient to area to schedule their medical consultation. 

5. Order Completion:
Once all steps are completed, the order is considered finalized.
The patient will subsequently meet with our medical team for further consultation and medication
dispensation.

Any necessary arrangements for shipping medication to the patient's home will be made at this stage.
Note: Ensure thorough adherence to each step of the SOP to maintain efficiency and accuracy in order
processing. Any discrepancies or issues should be promptly addressed and resolved to provide a
seamless customer experience.
